We are excited to have you join us at MDO!
Please complete the following Registration Forms and payment in order for your registration to be completed.
Registration Fee
An $80 Fee is due at the time of registration.
Payments can be made in the form of cash, check, or online.
Please bring cash or check to the MDO desk or mail to the P.O. Box listed at the bottom of this page.
If paying online, please specify the child's name and "Registration" in the notes and consider checking the box to cover the online payment fees.
Returning Form & Payment
Both the Registration Form and $80 Registration Fee must be recieved in order for your child to be on the registration list. Registrations will be taken first come, first served. Once classes are full, a waiting list will be established.​
